"Careful" used to mean "full of care" (anxiety).
In the NASB it is correctly translated "Be anxious for nothing".

Be anxious for nothing, but in everything by prayer and supplication with thanksgiving let your requests be made known to God. And the peace of God, which surpasses all comprehension, will guard your hearts and your minds in Christ Jesus. - Philippians 4:6-7 NASB

If careful means anxious. And it is saying be anxious for nothing. Isn't it saying to be careful in everything?

Actually it means to be carefree in the confidence that you are "in Jesus".
The best way to attain that is to read and study Paul's Epistles, as Paul
is the Apostle to the Gentiles. Don't be deceived, learn how to "rightly
divide" the Scriptures; they are not all written to you.
Anxiety comes when you are confused over the Scriptures and where you stand with God.
